When Anxiety Becomes Its Own Side Effect: Understanding the Nocebo Effect

We often talk about the placebo effect - how positive expectations can improve symptoms.

But there’s a quieter, less discussed counterpart that shows up frequently in anxiety disorders: the nocebo effect.


The nocebo effect happens when expecting harm or danger actually makes symptoms worse. In anxiety, this plays out not because something is physically wrong, but because the nervous system is already on high alert.


For someone with anxiety, thoughts like “What if this gets worse?”“What if I panic?”, or “What if this sensation means something serious?” don’t just stay in the mind. They activate the body. Heart rate increases. Breathing changes. Muscles tense. Sensations intensify. The fear is then “confirmed” by the body’s response.


This creates a powerful loop:

Anticipation → physical symptoms → interpretation as danger → more anxiety.


What’s important to understand is that this isn’t imagined or exaggerated. Research shows that negative expectations can heighten pain perception, increase stress hormones, and amplify bodily sensations. The experience is real - even if the trigger is psychological.


In anxiety disorders, the nocebo effect can show up as:

• Worsening symptoms after reading about side effects

• Increased panic after monitoring bodily sensations

• Fear of fear itself becoming the main driver of distress


This is why reassurance alone often isn’t enough. What helps is learning how to relate differently to sensations, thoughts, and uncertainty - so the nervous system doesn’t interpret every signal as a threat.


Sometimes, reducing anxiety isn’t about adding safety - it’s about removing the expectation of danger.
